Immunizations

The school do not hold or take student immunization record information - this information must be provided directly to Ottawa Public Health.  More information on providing this information can be found on the City of Ottawa's Partners and Community Organizations website.

Parents of new students registering for at school for the first time, are encouraged to provide the Immunization record for children as soon as is possible as soon as vaccinations take place and before attending school.  Please note that your doctors office will not update this information, it is the responsibility of parents to provide this information promptly to Ottawa Public Health directly.  All children attending school in Ontario must be immunized against diphtheria, tetanus, polio, measles and rubella, unless there is a valid exemption. Important Information Regarding OPH Immunization Surveillance 

Parents of students attending school, must provide full immunization information directly to Ottawa Public Health as soon as is possible, preferably before they attend school.  OPH has the authority to suspend students from attending school for whom OPH does not have a record of immunization. Important dates regarding OPH suspensions will be shared below as we receive information.  

Students in Grades JK to Grade 6 :
